Custom Field and Account Creation Questions

Hi. I want to use to mediate the communication with customers based on the level of my service (graphic design) they purchase.

For example there are three levels: silver, gold, and platinum

If someone buys the gold option I would like to send them to a specific page in kayako where they can create an account (within kayako). As part of the account creation there would need to be custom fields that they fill out specific to the gold option. If they purchase silver they would get sent to a different account creation page that has specific question only for the gold option. Is this possible??

Also, is there a custom field available so that one could attach or upload a file??
