This mod allows you to add unlimited custom fields for the LiveChat Leave a Message form. The data entered in this field is reflected in the Staff CP as well can be sent in the email which is sent to a forwarded email address (provided you have set the "Offline Messages Settings" in the Admin CP).

Yes you will have to email our Sales department. I would not prefer to upload the software source file in the forums. The mod is a combination of template and source modification allowing you to add custom fields in the Leave a Message form which is displayed to the clients when your LiveChat staff is Away/Offline or busy with other clients.

When I applied this mod to 3.11.01 the text is white and can't be seen unless you highlight it. Any idea why this might be?
