I highly recommend disabling toolbar/menu customization, it doesn't appear you're handling it correctly nor is it really necessary. |
Go into regedit and browse to HKEY_CURRENT_USER\Software\Kayako\LiveResponse. Then delete every sub-key that starts with "ChatFrame."
That should clear out the menu problem.
