"ERROR: No file was uploaded" in downloads section

ERROR: No file was uploadedi keep getting this error when i'm trying to upload a file in the downloads section. this is weird because i have successfully uploaded a file there in the past. this is only the second time i have used this function and nothing has changed since the first time i uploaded a file. the file i'm uploading is very small, only a 172kb

system specs
centos4.5
php5 5.1.6
mysql5

i have searched the forums and edited php.ini which has solved most people's problems.
here are the relevant settings in php.ini
max_execution_time = 1000
max_input_time = 120
memory_limit = 256M
post_max_size = 50M
file_uploads = On
upload_max_filesize = 50M
let me know if i'm missing any other info.

Take a look at Downloads over 2MB

Also, are you sure the files directory permissions are set to 777?

Which version of Kayako are you using? I believe this was a bug in Kayako.
