Insert Field Error 3.04.21

When I try to add a custom field I get the following error:

nvalid SQL: INSERT INTO `swcustomfields` (`customfieldgroupid`, `title`, `fieldtype`, `fieldname`, `defaultvalue`, `isrequired`, `usereditable`, `staffeditable`, `regexpvalidate`, `displayorder`, `description`) VALUES ('7', 'First Name', '2', 'd06b77a9b7', '', '1', '0', '0', '', '46', ''); (Unknown column 'description' in 'field list')
Warning: Cannot modify header information - headers already sent by (output started at /home/removed/public_html/helpcenter/includes/functions.php:410) in /home/removed/public_html/helpcenter/includes/functions_html.php on line 1371

Please make sure you downloaded the latest 3.04.21-CVS build and run the upgrade script. Please run upgrade script while setup this will reolve your issue.
