| Problem adding custom fields -

Perhaps someone on the forum can help us. We are running the latest version (3.30.02) of the SupportSuite and are trying to add additional custom fields to the user created support ticket.
Some time ago we created a group called 'Customer' and successfully added a field called 'Domain Name'. Now we are trying to add a second custom field for the customer's name. We click on 'Insert Field' in the menu and fill out the info. The system comes back with the insert successful message but the new field does not show up on the list of custom fields nor does it appear on the support ticket document.
We can go through the same process multiple times trying with new custom fields groups but it's always the same - the new field is not added.
Does anyone have any clues? Can the support ticket document be locked in some way so that it can't be changed. Please help!!
