You cannot modify the subject of the ticket. In doing so there is a good chance that you might break the email parser as well.

By at the top you mean in the email content?

You can add that by editing the email template under: Admin CP > Templates > Manage Templates > Tickets > email_* Professional and Affordable Kayako Skins - Specialists in Kayako Skinning & Customization - Professional Paid Support Our Skins and Services - http://www.supportskins.com/store/ - NEW SKIN ADDED!!
