Where to Insert Custom Field Type

Hi Guys,

I created a Custom field type named as ticket_voided.

Whent to CP ADMIN > TEMPLATES > Manage Templates > Tickets > submitticket.

At:
<{displaytemplate name="customfields"}>

And I got to insert under or below that line my example:
<input name="ticket_voided" value="">

First Question, is that correct ?

But that's not my point.

I got 3 departments.
- Sales
- Refund
- Airline Tickets

All of them, should ask you for that basics information like:
Full Name
Email
Subject
Message

But, the Refund, ask for more stuffs like:

E-ticket Number
Pax Name
Airline
And finally, my custom field type called Ticket Void (ticket_voided)

This field group should only appear at the refund. Ok, it's working as i defined at CP ADMIN.

But i don't know where to insert this INPUT ticket_voided together with other REFUND fields ?
