Maybe more of a company suggestion is to have a department that would work with companies in custom application design. Love kayako, but need some custom work done to help create a better flow and would be more than willing to pay for time, but was informed at this point, kayako does not offer this service.

There are a couple of companies that do this. Please check the market place board for more details.

Not in the sense I think that kayako as a company could perform. Such that we need sub departments built within kayako, and hence a kayako bid could be done to perform this task. Lets say I need kayako to coordinate with say a database such as an AS400, this would be something kayako would need to hard integrate.

Well I still think some of the 3rd party developers would be able to do this as you have 98% source code provided as you have the owned licence.
