I checked your phpinfo and found that its not showing MySQL. Please contact your sysadmin to reinstall your PHP with MySQL to fix this.

Some times its due to missing libmysql.dll or ext/php_mysql.dll. So, Please cross check these also.

@ taparikka, I had the same issue - I resolved it by changing short_open_tag to on in my php.ini

short_open_tag is set to On however as Mahesh pointed out, PHP is not compiled with MySQL support. You also need to change the settings I suggested.

PEBKAC on my part - I enabled the msql.dll plugin instead of the mysql.dll plugin. Saved php.ini, restarted IIS, and I was able to setup. Now on to my POP3 fetching problems... >.<
