| "Can't create table" error when upgrading -

We are trying to test upgrading our Kayako support site from our current live version (3.00.13 STABLE) to 3.04.10 STABLE - which are doing by creating a copy of our current live support site onto our internal test server and then performing the upgrade.
However, when trying to run the Update the stages all show a status (in red) of FAILED - showing an error message along the lines of:
Reason: Can't create table '.\<database name>\swfloodhash.frm' (errno: 121)
If I go back and try to run the update again, the list of versions the update is going to go through no longer shows the one that was first in the list, and trying to update shows the same error again, but with a different table name each time.
Interestingly, if I view the test support site once updated it shows the correct version number, and looks as I would expect - but when I try to submit a test ticket in the customer view or perform a ticket search in the staff view I get "Invalid SQL" errors along the lines of:
Unknown column 'lastuserreplytime' in 'fieldlist'
Has anyone seen this behaviour before?
Any ideas on what we can do to resolve this?
I'm reluctant to run the update on our live server until this is resolved as we cannot afford to have any downtime should it fail.
