Kayako Helpdesk link not working

I have followed the integration instructions provided using so many different combinations. But cannot get the helpdesk/support link working after a client logs in and clicks on the support link.

• Has anyone else had this problem and resolved it?
• Is there a simple reason why this integration does not work with MB5?
